This subject would be a lot easier to understand if CCP used the correct terminology.
Medical Clone = your respawn point.
Upgrade Clone = insure your character's SP amount - it is not done to an individual clone and you don't upgrade anything. I emphasis it is a character wide insurance and covers all Jump clones, but only for a single death.
Jump Clone = simply a copy of you which starts with no implants and which you can insert a different set (of implants). Note that you can only jump (change clones) once every 24 hours.
In the beginning there is you. You start and will always have a default insurance to cover 900000 SP. As your SP increases beyond that default level, you need to (upgrade clone) pay for insurance to cover the larger amount.
When you die, you respawn (medical clone) with the same SP, provided you had adequate insurance (upgrade clone). If you didn't have adequate insurance you lose 5% of the uninsured SP.
The insurance lasts for one respawn only, so if you have 5M SP and take insurance for 20M SP, then die, you respawn with 5M SP and no insurance other than the default 900K. If you die a second time without reinsuring you will lose 5% of 4.1M SP, namely 205K and will respawn the second time with 4795000 SP. The SP is taken from the last Skills you trained (I believe). It is common to forget to immediately reinsure (upgrade clone) after death and then lose SP on a second death.
After meeting a few prerequisites, you can create one or more Jump Clones. Jump Clones are used for two reasons.
1. To relocate from one part of the Eve Universe to another, without having to fly between the two points. This may be desired to avoid long distance flights or simply between High / Low / Null secs when you want to avoid going through gate camps. Note though that only you jump, not your ship or your cargo, they have to be taken the hard way.
2. To perserve or utilise different sets of implants. In a high risk situation for example, you may want to switch to a clone with cheaper implants or combat related implants.
